On your device. Where an app offers iCloud sync it runs through your own iCloud account. Each app's linked page states what that app does with your data.
I paid on one device. Do I pay again on another?
No. Purchases are tied to your Apple Account, so restoring on another device signed in to the same account brings the purchase with it.
Refunds
Refunds for App Store purchases are handled by Apple, not by me. Use reportaproblem.apple.com with the Apple Account you bought with.
